Join us on December 17th to learn strategies to prevent medication errors, when to use root-cause analysis and how to incorporate failure-mode-and-effects analysis to prevent errors from happening.

Review strategies for conducting an efficient medication reconciliation on transitions of care to reduce medication errors.
Give examples of common errors discovered during a medication reconciliation.
Describe root cause analysis and failure modes and effects analysis (FMEA) and their roles in preventing medication errors.
Identify error-prone situations and processes to improve patient outcomes.

Instructor Bio: Dana Saffel, PharmD

Dana Saffel, PharmD, DPh, CGP, FASCP, is President and CEO of PharmaCare Strategies, Inc., a market development firm that specializes in assisting pharmaceutical manufacturers and pharmacy providers in positioning key products in specialty channels such as long-term care, managed care, Medicaid/Medicare and hospital markets. As a part of this role, Dr. Saffel provides market development consulting on contracting
strategies, pull-through initiatives, advertising, and product placement to compliment both existing products as well as potential products in development.

Additionally, Dr. Saffel moderates industry-related advisory boards, and provides educational programming to healthcare professionals. Dr. Saffel is a frequently lecturer at both national and regional meetings on geriatric disease management and health care policy including Medicaid and Medicare. Prior to starting PharmaCare Strategies, Dr. Saffel was Vice President of United Pharmacy Services, a long-term care pharmacy serving over 12,000 NF, ALF, correctional and developmentally disabled residents throughout Georgia, North Carolina, and South Carolina. S

Dr. Saffel was among the first group to successfully complete Certification in Geriatric Pharmacy. Dr. Saffel is active in many areas of senior-focused professional organizations such as ASCP, AMDA, and AGS. At ASCP she has served on the Public Affairs Council and Government Affairs Council and the Board of Directors of ASCP’s Research and Education Foundation and is a past member of the Board of Commissioners for Certification in Geriatric Pharmacy. She is also active in the Florida Pharmacy Association and is a past-president of the Florida Chapter of ASCP.

Moderator Bio: Pamela Bryan Kramer, BA, LPhT Bio

Pam Kramer has been involved in process development and improvement in every company with whom she has been associated during her more than 35 years in the pharmaceutical industry. She has worked across the country with agencies, hospitals, healthcare centers, and pharmaceutical/biotechnology manufacturers, creating award-winning strategies and programs in oncology, cardiology, infectious diseases, pharmacy, and more for such noted clients as Abbott Laboratories, Parke-Davis, and Genentech. She joined Forum in 2004 as Director of Marketing & Client Relations, working with client facilities to align pharmacy services to customer needs. With a unique background that balances strategic planning and operational process development with a keen understanding of what her customers need, she was named Vice President of Business Operations in 2007 and Executive Vice President in 2011. Responsible for development and implementation of Forum’s overall company strategy, as well as overseeing pharmacy and business operations, she is intimately involved in optimizing infrastructure and services to create exceptional value for customers. Pam also serves as the pharmacy’s lead educator, developing programs on compliance, controlled substance regulations, reimbursement, industry trends, process development, and procedural improvement. With particular expertise in electronic health records and reimbursement, Pam has been a speaker at educational meetings, family nights, and conferences such as the American Society of Consultant Pharmacists, Illinois Health Care Association, and the Leading Age, Illinois. She holds a Bachelor’s degree from New York University and is a Licensed Pharmacy Technician.
