The Boy Scout motto “Be prepared” is also the mantra for every long-term care facility and assisted living community as they prepare for their surveys. The goal of every leadership team is to be survey ready, every day. Medication management is incredibly important as it has implications throughout the survey, from medication administration reviews and storage to electronic health record completeness and accuracy. And throughout the process, surveyors are looking for medication errors.

This “don’t miss” session will provide insight and best practices for gold-standard survey preparedness that can be adapted for every license level, as well as useful tactics during the actual survey process.

With more than 20 years’ experience in pharmacy and having spent the last 15 in long-term care, Johnson Abraham manages Forum’s clinical programs and consultant services. In his role he works directly with facility management to ensure that our services meet facilities’ clinical needs and align with desired outcomes. He joined Forum in 2007 as a Consultant Pharmacist from Enloe Drugs of Northern Illinois / Omnicare, and was promoted to Director of Clinical Services in 2014.

A Board-Certified Geriatric Pharmacist, Johnson received his Doctorate in pharmacy from the University of Illinois at Chicago after Pharmacy and Business Management internships with Walgreens. He is a former President of the Illinois Chapter and member of the American Society of Consultant Pharmacists.

Barbara Hackel brings more than 35 years of experience in a range of positions in long-term care, home health, and acute care to Forum. Having worked at Resurrection Health Services, MacNeal Home Care, Evanston Hospital, as well as an Assistant Director of Nursing and Care Manager for Lexington Health Care, Barbara has a deep understanding of her customer’s clinical operations.

A graduate of the Ravenswood School of Nursing, Barbara received her Bachelor of Science in nursing from Lewis University. She is PICC-line certified and a member of the Infusion Nurses Society.

Peggy Weder’s career in long-term care, home health, and acute care spans more than 35 years and includes 19 years with Forum. She has held a range of positions with Provena St Joseph Medical Center, University HealthSystem Consortium, as well as serving as the Director of Nursing at Rosewood Care Center. Her insights and expertise both from a clinical and business standpoint provide broad value to her customers.

After graduating from the St. Joseph College School of Nursing, Peggy received her Bachelor of Science in nursing from Olivet Nazarene University. She also received her Master’s in Business Administration from the University of St Francis. Peggy is PICC-line certified and a member of the Infusion Nurses Society.

Pam Kramer has been involved in process development and improvement in every company with whom she has been associated during her more than 35 years in the pharmaceutical industry. She has worked across the country with agencies, hospitals, healthcare centers, and pharmaceutical/biotechnology manufacturers, creating award-winning strategies and programs in oncology, cardiology, infectious diseases, pharmacy, and more for such noted clients as Abbott Laboratories, Parke-Davis, and Genentech. She joined Forum in 2004 as Director of Marketing & Client Relations, working with client facilities to align pharmacy services to customer needs. With a unique background that balances strategic planning and operational process development with a keen understanding of what her customers need, she was named Vice President of Business Operations in 2007 and Executive Vice President in 2011. Responsible for development and implementation of Forum’s overall company strategy, as well as overseeing pharmacy and business operations, she is intimately involved in optimizing infrastructure and services to create exceptional value for customers. Pam also serves as the pharmacy’s lead educator, developing programs on compliance, controlled substance regulations, reimbursement, industry trends, process development, and procedural improvement. With particular expertise in electronic health records and reimbursement, Pam has been a speaker at educational meetings, family nights, and conferences such as the American Society of Consultant Pharmacists, Illinois Health Care Association, and the Leading Age, Illinois. She holds a Bachelor’s degree from New York University and is a Licensed Pharmacy Technician.
